I Died... don't know why

so, on 2-12-17 at about 9:45pm eastern standard time (usa) I was in the nether at my fort, doing good, full health, top quality gear standing in a wide open area with no mobs near... when I died.

This wasn't even an attack by a bad player.. no lava or anything
My screen went black and I started taking damage rapidly and was dead within maybe 2 seconds.
It was like I fell into the void... but I was no where near anything to fall into.

What happened? Any hope to get the stuff back... prolly not but ya gotta ask, right?

Can you go again to the area where you died? When I come to survival I want you to show me.

You can open a ticket and ask to check the PAC Records. Big servers like Pika should have logs that store damage logs (unless they are so lazy they don't even have such plugins).
